Stage I. Preparation for IVF Program - Services: *medical examination of genetic father or family couple; *consulting services provided by reproductologist, andrologist, gynecologist and other specialists; *all medical tests should be conducted on orders of “La Vita Nova” doctor; *selection, medical testing of surrogate mother and, if necessary, oocyte donor, their preparation for IVF program; *approval of surrogate mother/oocyte donor for IVF program./ Stage II. – 1st part of IVF + ICSI - Services: *preparation of surrogate mother’s endometrium for transfer of embryo/embryos; *synchronization of menstrual cycles of surrogate mother and genetic mother/oocyte donor; *stimulation of superovulation of ovaries of genetic mother/oocyte donor, prescription of medicines for stimulation of oocyte donor’s ovulation./ Stage III. – 2nd part of IVF + ICSI - Services: *retrieval (puncture) of oocytes of genetic mother/oocyte donor; *application of PICSI procedure on orders of reproductologist; *fertilization of oocytes with genetic father’s spermatozoa; *generation of embryos and their cultivation during 3-5 days; *transfers of all healthy embryos into uterine cavity of surrogate mother; quantity of embryos to be transferred each time should be chosen by reproductologist; *1st pregnancy test; *2nd pregnancy test; *3rd pregnancy test, confirmation of heartbeat./ Stage IV. Maintenance of pregnancy till week 28 - Services: *control over surrogate mother during fetal pregnancy period./ Stage V. Maintenance of pregnancy after week 28 - Services: *control over surrogate mother during obstetrical pregnancy period./ Stage VI. Labor/childbirth in Kharkov, Ukraine. Legal and guest services - Services: *delivery at Kharkov maternity hospital; *registration of newborn child, issuance of Ukrainian Birth Certificate and Extract from Stage Register, their apostillization, translation and notarization; *accommodation for period of 5 days; *transfer from airport to place of accommodation on day of arrival and back to airport on day of departure; *examination of child by pediatrician on day of discharge from hospital and one day before departure from Kharkov.
