Explain your reasons for wanting a surrogate/donor:
I have gotten pregnant many times but sadly each pregnancy has ended in a miscarriage. After seeing multiple doctors, including miscarriage specialists, I finally turned to IVF. To date, I've done 3 transfers and each has resulted in a miscarriage. Throughout this journey, none of the doctors can explain why this is happening. We have 5 genetically tested embryos on ice and have decided that it's now time to turn to surrogacy.

We have been married for 10 years and for the last 5 years, we have been trying to start a family. We are both professionals and work and live in downtown Chicago. Since I was a little girl, I've wanted to be a Mom. After years of trying to have this dream fulfilled I now realize that I need help with this.
