Explain your reasons for wanting a surrogate/donor:

I myself have children from a previous marriage, but due to medical complications am now infertile. I remarried a few years ago, and my husband and i want to complete our family with a biological child of our own.

How would you best describe yourself(s):

I am a very physically active, and energetic nursing student (graduating next year) in Ontario. I am very open minded and have a unique sense of humor.

Use this space to communicate a letter to potential Surrogates/Donors:

I understand that this is a unique situation, and that it is important that IP's and TS match and are compatible. I hope that you take the time to get to know myself, and my husband and consider helping us complete our family. We are financially stable, fun loving, amazing parents and have so much to offer. we're open to as much, or little involvement from TS as they're comfortable with. As I said before, we are very open minded, and would love to get to know you!
