Explain your reasons for wanting a surrogate/donor:
My husband and I have been trying to conceive a child of our own for over 4 years / I have an extreme case of PCOS. We have done many rounds of fertility treatments, with no luck. Due to a medication error, I ended up having 3 surgeries on my ovaries in a very sthort amount of time, knowing each surgery drastically reduced our chances of being able to conceive.
