Explain your reasons for wanting a surrogate/donor:

The mother (egg donor) is a life long friend. Married with children of her own. I'm single, for some years now and cannot afford to wait for right individual to appear, hence intention to father a child alone. Looking for surrogate mother willing to bring child to term and provide breast milk for immediate weeks following birth. Myself: Blood group B Neg Egg donor: Blood Group O Neg Basic requirements/questions: 1. Health checks including mental health.  2. As a surrogate, do you have the support of your family? 3. Are you religiously neutral on the topic of surrogacy. 4. I would require some contact with the surrogate's husband, partner (if applicable). 5. If there has been previous pregnancies, I would want to see the medical notes. How did the other births go?  i.e. Full term?  Natural?  Drugs used? Post Natal care? etc 6. During the first stages of the pregnancy, if test's show there is any abnormality/defect that would impede the development of the foetus and the well-being of a possible child I and the mother (egg donor) would require a termination of the pregnancy. 7. I would require to be present at the birth. 8. Ideally I would want any child as the result of this arrangement to be able to contact the surrogate occasionally to send holiday photos/cards, but if no contact is wanted that is okay. 9. Review of current healthcare plan and some possible contribution to upgrade the plan. Thanks for your time and understanding...
