Explain your reasons for wanting a surrogate/donor:

In May of *** I was unexpectedly diagnosed and treated for stage 2 breast cancer.After a mastectomy, chemotherapy and reconstructive surgery, my life was basically returned to normal a year later, aside from the deep learning and wisdom I gained from the experience that has impacted my life for the better. The effects of chemotherapy left me without the ability to have children naturally.However, through the miracle of modern medicine, my husband and I were able to use an anonymous egg donor to create embryos with my husband’s sperm to get pregnant with our twin boys.They were born 6.5 weeks early after a rough pregnancy that included gestational diabetes, pre-eclampsia and post pregnancy issues with cardio-myopathy.My health has returned to normal, and I am no longer taking heart medications, but my doctors have strongly advised that if I were to get pregnant again, the cardio-myopathy would most likely return and be a risk to my life and the life of any fetus. It is for this reason that my husband and I are unable to conceive another child by traditional means.We have three high quality, female embryos left from the IVF process we went through to get our twin boys.We feel strongly that there is a little girl to join and complete our family.We have decided that a surrogate pregnancy is the best choice for us.My physician confirms that I am in good health, am not under treatment for any medical conditions, have no infectious diseases.

How would you best describe yourself(s):

IM is kind, compassionate and creative. She is an excellent mother and it shows in how happy our twins are. She waited a long time and has faced and overcome many obstacles in her life. She never gives up and sacrifices and serves me and our children. IP is proactive, fun and extremely hard working. He is great with kids and always has new ideas about how we can help them learn and grow. He is a good provider for our family and supports me and the kids in all that we do.
