Explain your reasons for wanting a surrogate/donor:

After a terrifyingly difficult pregnancy I was told a second pregnancy could be fatal - certainly much more life threatening than my first. As a result, I elected to have a tubal ligation to prevent further pregnancies and have been pursuing completing my family via surrogacy.
