Explain your reasons for wanting a surrogate/donor:

I have been through several different types of fertility treatments over the course of approximately 3 1/2 years, with one miscarriage and then no further pregnancy success. I am now 37 years old and the likelihood of having a full term pregnancy may not be an option for me. Therefore, we are considering a surrogate who would use my egg and Jay's sperm.

How would you best describe yourself(s):

My husband Jay and I are both loving, kind, family-oriented people. We have known each other since childhood and have built a strong life, faith, and extended group of family and friends over the years. Our families live nearby so we celebrate birthdays and holidays with parents, siblings, and extended family. We live in the country and enjoy the outdoors. Jay was an athlete growing up, and will likely coach any sport or activity our child will engage in. I am a mental health therapist and will likely be the mom on the parent school board, cheerleading coach, and will worry all day before he or she takes the road test! We hope to have the opportunity through the assistance of a surrogate to have a beautiful little one of our own. We feel so blessed with everything we have, though the greatest gift of all would be to have someone work with Jay and me so that we can fulfill our dream of having a baby!!!
